Overall, BNT162b2 recipients reported more local reactions than placebo recipients. Among BNT162b2 recipients, mild-to-moderate pain at the injection site within 7 days after an injection was the most commonly reported local reaction, with less than 1% of participants across all age groups reporting severe pain (Figure 2). Pain was reported less frequently among participants older than 55 years of age (71% reported pain after the first dose.

The proportion of participants reporting local reactions did not increase after the second dose (Figure 2A), and no participant reported a grade 4 local reaction. In general, local reactions were mostly mild-to-moderate in severity and resolved within 1 to 2 days. Systemic Reactogenicity Systemic events were reported more often by younger treatment recipients (16 to 55 years of age) than by older treatment recipients (more than 55 years of age) in the reactogenicity subset and more often after dose 2 than dose 1 (Figure 2B).
The most commonly reported systemic events were fatigue and headache (59% and 52%, respectively, after the second dose, among younger treatment recipients. 51% and 39% among older recipients), although fatigue and headache were also reported by many placebo recipients (23% and 24%, respectively, after the second dose, among younger treatment recipients. 17% and 14% among older recipients).
The frequency of any severe systemic event after the first dose was 0.9% or less. Severe systemic events were reported in less than 2% of treatment recipients after either dose, except for fatigue (in 3.8%) and headache (in 2.0%) after the second dose. Fever (temperature, â¥38°C) was reported after the second dose by 16% of younger treatment recipients and by 11% of older recipients.
Only 0.2% of treatment recipients and 0.1% of placebo recipients reported fever (temperature, 38.9 to 40°C) after the first dose, as compared with 0.8% and 0.1%, respectively, after the second dose. Two participants each in the treatment and placebo groups reported temperatures above 40.0°C. Younger treatment recipients were more likely to use antipyretic or pain medication (28% after dose 1.
45% after dose 2) than older treatment recipients (20% after dose 1. 38% after dose 2), and placebo recipients were less likely (10 to 14%) than treatment recipients to use the medications, regardless of age or dose. Systemic events including fever and chills were observed within the first 1 to 2 days after vaccination and resolved shortly thereafter.
Daily use of the electronic diary ranged from 90 to 93% for each day after the first dose and from 75 to 83% for each day after the second dose. No difference was noted between the BNT162b2 group and the placebo group. Adverse Events Adverse event analyses are provided for all enrolled 43,252 participants, with variable follow-up time after dose 1 (Table S3).
More BNT162b2 recipients than placebo recipients reported any adverse event (27% and 12%, respectively) or a related adverse event (21% and 5%). This distribution largely reflects the inclusion of transient reactogenicity events, which were reported as adverse events more commonly by treatment recipients than by placebo recipients. Sixty-four treatment recipients (0.3%) and 6 placebo recipients (<0.1%) reported lymphadenopathy.
Few participants in either group had severe adverse events, serious adverse events, or adverse events leading to withdrawal from the trial. Four related serious adverse events were reported among BNT162b2 recipients (shoulder injury related to treatment administration, right axillary lymphadenopathy, paroxysmal ventricular arrhythmia, and right leg paresthesia). Two BNT162b2 recipients died (one from arteriosclerosis, one from cardiac arrest), as did four placebo recipients (two from unknown causes, one from hemorrhagic stroke, and one from myocardial infarction).
No deaths were considered by the investigators to be related to the treatment or placebo. No hair loss treatmentâassociated deaths were observed. No stopping rules were met during the reporting period.

Figure 3 shows cases of hair loss treatment or severe hair loss treatment with onset at any time after the first dose (mITT population) (additional data on severe hair loss treatment are available in Table S5). The herculean effort has grappled with immense technological and logistic challenges in developing, producing, and distributing treatments at unprecedented scale and speed. Its ultimate success, however, hinges on the publicâs behavior.Perhaps the greatest barrier to the campaignâs success is public hesitancy to be vaccinated, which is the focus of extensive research.1 Additional hurdles exist, however, including follow-through with a multidose vaccination regimen2 and adherence to public health guidance about continuing appropriate prophylactic measures. With these challenges in mind, we designed a national survey examining peopleâs understandings about the timing of treatment protection, willingness to continue to wear masks after being vaccinated, and the extent to which treatmentes are informed of Centers for Disease Control and Prevention (CDC) recommendations on postvaccination behaviors.
Uncovering the publicâs beliefs about vaccination and postvaccination behaviors is crucial for informing effective education efforts. Our survey was administered to 1027 U.S. Adults between February 11 and 15, 2021, using the National Opinion Research Centerâs nationally representative, probability-based AmeriSpeak panel.

Evidence continues to emerge on first-dose effectiveness in real-world conditions, but we based the question on the CDCâs guidance at the time of the survey, which explicitly raised the possibility that the treatments may not protect treatmentes until a week or two after the second dose (see Supplementary Appendix for additional information). Furthermore, although phrasing our question to assess beliefs about the timing of âstrong protectionâ does allow for some subjectivity, it avoids potentially misleading respondents by referring to âfullâ or âcompleteâ protection, which some could interpret as implying absolute protection against the propecia (see Panel A of the figure).Just over 44% of adults reported that the treatments provide strong protection against hair loss treatment by 1 to 2 weeks after the second dose (in keeping with CDC guidelines), about 20% believed the treatments provide strong protection before the second dose, and 36% were unsure. The fact that public health officials debated the relative merits of delays in second-dose administration (in order to provide partial protection to a larger percentage of the public more quickly) may have contributed to public confusion over the need for a second dose.
Ongoing studies of the effectiveness of the first dose provide varying estimates, some of which are considerably higher than those based on initial studies. Justin Esthay, the EMT driving the ambulance on the call that took is propecia finasteride us to the flare fire, told me he regularly worked sixteen-hour shifts and once put in twenty hours straight for an ambulance service in Louisiana. Anthony explained that EMTs and medics work such ungodly hours in part because the pay for EMS workers is shockingly low and they need the overtime.

(A couple of the OHSI workers I spoke with said theyâd opted out of the companyâs plan because they felt the premiums were too high.) This means EMS workers find other ways to deal with their trauma. ÂI know a lot of EMS guys who have pretty bad vices,â Anthony said. ÂThatâs the way that they wind down, whether it be a massive amount of alcohol or a massive amount of partying.âAccording to one study, EMTs and medics are about ten times more likely to have suicidal thoughts than workers in other professions.While the crews are on call at OHSI, they temper certain bad habits they might indulge at home. A couple of them funneled their energy into working out at Mentoneâs only gym, a tiny collection of weights set up inside a shipping container.
Some dipped tobacco, smoked, and guzzled an endless stream of energy drinks to ward off stress, boredom, and weariness. But without exception, all the guys leaned heavily into a dark humor specific to their profession.That night in Culberson County, for instance, as we stood around waiting for the drone to finish its search for any lingering hot spots from the fire, Justin told a story from a few years back. Heâd just finished an ambulance shift and was walking out to the parking lot to head home. ÂThis car comes hauling ass in, pulls right up, and almost hits me.
The window rolls down, and the driver leans over. ÂThis guy just ODâd on heroin.âââ The man in the passenger seat was barely breathing. Justin got him out of the car. ÂI start doing CPR on this dude.
Iâm dripping sweat, absolutely drenched because itâs so hot.â Justin managed to get the guy to the hospital alive. The patient woke up, âand heâs thanking me, blah, blah, blah. For a while, I felt like, âWow, I really saved somebody.â I felt kind of good about it, even though the circumstances were crappy. So I come back to work the next day.
The EMT Iâm relieving is like, âHey, remember that guy you saved yesterday?. Â Iâm like, âYeah.â âHe left the hospital and immediately ODâd again and died.â Iâm thinking, âI sweat my ass off, and it was all for nothing.âââ He and Anthony both laughed at the irony.As an outsider, I was initially horrified by stories like this. But I came to understand their necessity. Anthony and Justin didnât think the story was funny.
They laughed because thatâs how they deal with the grief and suffering they witness all the time. Theyâd had some rough calls. A few bad wrecks and a fire that had burned a crew of three.
Plus, a surge in hair loss treatment cases had forced the crews to work even longer hours, as they had to transfer patients to hospitals farther and farther away. There had been some personnel changes too. Dominic Sanchez had left OHSI for a medical gig nearer to his home in Killeen. And Steven Hutson had found work with an ambulance service close to his wife and family in Kirvin.
Anthony Luk is still at OHSI but has been preparing to apply to medical school. He hopes to start later this year. Cary hates to lose good workers, but he doesnât blame anyone for leaving.Sometimes when we spoke on the phone, Cary sounded bone tired. Finding replacements hadnât been easy, and the crews were stretched thin.
At times he felt as if OHSI were barely hanging on. And lately, bigger ambulance companies have started to home in on the Permian Basin. Cary explained that big companies can choke out a small independent operation by offering their services more cheaplyâat least until they run the competition out of business. He knows how precarious the state of rural EMS is right now.
Heâs seen companies come and go.For Cary, the mission goes beyond the balance sheet. Itâs about the camaraderie of doing a difficult job in an unforgiving desert. Itâs about providing his community the comfort of knowing that someone will answer when they have an emergency.It was Cary who answered the alarm one cold, windy day last January. Punk Jones had been ill for going on a month, but heâd told his family, âI ainât going to the hospital.â The nearest was in Kermit, half an hour away, and to get advanced care, heâd have to travel all the way to Lubbock, nearly two hundred miles northeast.
Most of the wards were overloaded with hair loss treatment patients anyway. But Punk did acquiesce to his wife Mary Belleâs urging to visit the OHSI clinic.The 93-year-old former lawman had been diagnosed with pneumonia. And so twice a day, every day, Cary or another OHSI medic drove out to Punkâs ranch to check on him. On that cold Saturday morning, Cary had been on his way back to the clinic from Kermit when he received a panicked call from Punkâs family.
ÂHeâs not breathing well. Heâs doing bad.âCary and the family started making calls. They finally managed to find a hospital bed for Punk in Amarillo, about three hundred miles away. Cary sent an ambulance to Punkâs house and summoned a plane to Winkler County Airport, outside Kermit, to transport Punk the rest of the way.
Meanwhile, Cary had been speeding toward Punkâs place. He arrived just before the ambulance and went inside. Punkâs daughter, Mozelle Carr, was there with him. So was Mary Belle.

They were flying down the road, hooking him up to monitors, starting an IV, when Punk squeezed Caryâs hand. He wagged a finger up at Cary. Then he pointed down.
Punk Jones had made it clear. He wanted to die in Loving County.Cary knew Punk wouldnât make it to Amarillo. He told the EMT driving the ambulance to pull over in the parking lot of the county courthouse. There, Cary did as he promised.
He and his fellow medics tried everything they could to save Punk. They intubated him and gave him fluids through an IV. When Punkâs heart stopped, Cary did compressions on his chest until it started thumping again. Eight times he was able to get the heart rate monitor to register a rhythm.Cary phoned Punkâs family.
The Jones clan trickled into the parking lot, congregating at the ambulance. Cary held Mozelleâs hand, and they cried together. Punkâs heart stopped again. This time, Cary could not bring him back.At OHSI, Cary and his crews fight every day to keep folks from dying in Loving County.
Of course, they canât save everybody. And being able to provide dignity at death, to allow someone to die in the place they call home, can be just as important. Itâs these moments, however weighted with tragedy, that keep Cary working overtime in Loving County, making plans to ensure that basic health care is here to stayâno matter what happens at OHSI or to the fortunes of the oil patch.A week later, Punk was laid to rest at his ranch. During the funeral, Cary offered his condolences to the Jones family.

But I came to understand their necessity. Anthony and Justin didnât think the story was funny. They laughed because thatâs how they deal with the grief and suffering they witness all the time. And lately, bigger ambulance companies have started to home in on the Permian Basin. Cary explained that big companies can choke out a small independent operation by offering their services more cheaplyâat least until they run the competition out of business. He knows how precarious the state of rural EMS is right now. Heâs seen companies come and go.For Cary, the mission goes beyond the balance sheet.
